Telehealth Requirements
Patients must participate from a private, safe location with reliable internet access. At the beginning of every telehealth visit, patients must provide their current physical location (address or identifiable location) so emergency services can be dispatched if necessary. Patients must also provide and maintain an up-to-date emergency contact.
Safety
Patients may not participate in telehealth appointments while driving, operating machinery, or engaging in any activity that could compromise safety. If a provider determines that a patient is driving or otherwise unable to safely participate, the visit may be ended and rescheduled.
Emergencies
Mindful Vitality Partners is an outpatient psychiatric practice and does not provide emergency or crisis services. If there is an imminent risk of harm to yourself or others during a telehealth visit, the provider may contact emergency services or your designated emergency contact when clinically appropriate.
Additional Information
Telehealth services are available only when clinically appropriate and only in jurisdictions where your clinician is authorized to practice. A provider-patient relationship is established only after evaluation, acceptance into care, and completion of required documentation.

Telehealth Services
Telehealth appointments are offered when clinically appropriate and permitted by applicable law.
Patients receiving telehealth services must:
Be physically located in a state where the treating clinician is authorized to practice.
Participate from a safe, private location.
Provide their physical location at the beginning of each telehealth visit.
Provide an up-to-date emergency contact.
Not participate while driving, operating machinery, or engaging in activities that may compromise safety.
If a provider determines that a patient cannot safely participate in a telehealth visit, the appointment may be terminated and rescheduled.
